Update to previous cycle's plan for continuous improvement Again, the correct questions did not reflect the information we were seeking on the End of Year survey.  This is quite a lengthy survey and is sometimes difficult to manage.  We will try to do a better job reviewing questions prior to its release at the end of the year - 2015.

We have secured an additional RHD position, bringing the number to 8.

We did a CAS review this year, but it was after the goals were developed.  We are currently working on next year's goals and will use CAS.

Plan for continuous improvement We will continue the quest to complete our reorganization plan.

We intend to re-write our end-of-year survey to better reflect information needed for our goals, and will use CAS to write our goals.
